This evaluation will assess a unique medical home payment model under which primary care practices are paid a comprehensive, risk-adjusted, per-patient annual fee covering infrastructure and salaries. Currently, this global fee model is being tested as part of a medical home demonstration in nine primary care practices in Albany, N.Y., and Massachusetts. The proposed evaluation will determine if the new payment method is associated with reduced health system costs, improved patient care outcomes, and higher patient and physician satisfaction. Cofunding is being sought from the Hatch Family Foundation.
